This page provides access to Roundtable forms below.  

 Forms highlighted in blue are standard forms.  They can be  downloaded, printed, filled out manually and mailed to the address on the form.

Forms in green are “fillable” forms, online entry forms.  

You must use Adobe Acrobat Reader to open the “fillable” forms.  Adobe Acrobat Reader is free to download and install from this site:  click here.  

Once Acrobat Reader is installed, follow this 3-step process, to process the form.

  1. To download form: hover the cursor over the form name, right click and “save link as…” and save the document to your desktop.    

  2. To open form – locate the document on your desktop, right click the form and “open with”  Adobe Acrobat Reader  

  3. To update form: type the necessary information in the form and save it to your desktop with a name you will remember.

  4. Email the saved form as an email attachment to Forms@thecommunityroundtable.org  using your normal email tool, such as Outlook, Gmail, Yahoo, Verizon.net, etc…

    Note:  Instead of mailing a check, you can pay application/registration fees online at the Roundtable Store through secure and safe Paypal.  A Paypal account is not needed.   We accept credit cards!
